Once you’ve installed the plugin, you’ll find a new icon appears above your editor so click it.

After click this icon, you’ll see a pop up window with the form you can easily customize.

Can i add more fields?

Yes you can, Simply click the click here and start adding your own custom form fields.

Its just too easy!

You can drag and drop each field into the position you want and change the name of the fields.
